Dear Diary,
I don't know what's worse: pointing fingers at each other or blaming both of ourselves accountable for what happened and too ashamed to be the first one to address the problem.

I have experienced this situation. I think its always better to be the first one to show courage. Accepting there was fault on my part and saying it loud takes the weight off your mind amd conscience.
